Responsibility 1. Personal Care
Provides personal care to the patient, which includes the following activities in accordance with the care plan. (see attached for detailed activities)
Responsibility 2. Lifts and Transfers
Utilizes proper body mechanics and transfer techniques in order to carry out safe lifts and/or transfers (see attached for detailed activities)
Responsibility 3. Nutrition/Food/Storage
Prepares nutritious, varied, economical meals/snacks, including special diets as determined by the care plan; uses proper food preparation, storage and hygiene practices. (see attached for detailed activities)
Responsibility 4. Cognitive, Social and/or Emotional Support
Provides cognitive, social and/or emotional support to the patient. (see attached for detailed activities)
Responsibility 5. Adaptive Aids
Provides assistance with the use and care of adaptive aids. (see attached for detailed activities)
Responsibility 6. Home Management
Provides assistance with a variety of household tasks. (see attached for detailed activities)
Responsibility 7. *Medication
Provides assistance with medications. (see attached for detailed activities)
Responsibility 8. Palliative Care
Applies concepts of palliation and provides care after death.(see attached for detailed activities)
Responsibility 9. Infant/Child Care
Provides infant and child care. (see attached for detailed activities)
Responsibility 10. Maintains a Safe Environment
Promotes a safe environment for both the patient and worker.
Understands that reporting safety issues is encouraged as a crucial step toward improvement. (see attached for detailed activities)
Responsibility 11. Reporting and Communication
Observes and reports any changes or problems related to the patient or patient’s environment to the immediate Supervisor in a timely manner. (see attached for details)
Majority of the areas require workers with driving silence and reliable car.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
The minimum qualifications for this position include completion of the Personal Support Worker training Program, which includes Modules 1-14 theory and the required practicum component as identified in the training document developed by the Ministry of Health, Long Term Care Division (1997).
* Interpersonal skills typically gained through daily living skills acquired in caring for the home and family, elderly, disabled or ill family members with a variety of health care needs.
*Upon hire must obtain a copy of the of PSW certificate.
